Requirements for Annual Participation

This needs to be completed before participation in any practices:

• An updated physical
• Completed Health Screening Form - to be filled out online - Any questions contact the MSHS Nurse's Office
• Take Impact Baseline Test

This needs to be completed before participation in any scrimmages or game:

A sports information meeting is held every August prior to the school year. All athletes and parents/guardians are encouraged to attend.
